As of March 1, 2010 all clinical offices & local services of ACOS / provider(s) have closed. The terms of closing were voluntary and in preparation for the system-wide governmental changes in health care law.
In accordance with state guidelines, all medical records remain on file and can be obtained by completing the following steps:
1. Provide a legible written request for medical records letter to the ACOS mailing office.
2. Provide a legible full legal name of the patient with their legal date of birth.
3. Provide a legible signed authorization letter for medical records by the patient or legal guardian.
4. Provide a legible name & address of the recipient for the records to be sent if not the patient.
5. Enclose the $25 fee for each medical record request; payable by check, cash or money order payable to ACOS.
Neither ACOS or any of it's providers are currently providing general primary non-military surgical services in Arizona at this time. Non-clinical Administrative / Consultation services will be handled on a per project basis.
